1. Purchase qualifying Simrad NX40 during the valid mail-in rebate offer period April 1, through August 31, 2009.
2. Complete both sides of this rebate form. Customer must sign the agreement on form.
3. Mail the completed form, along with sales receipt and completed Warranty Registration form, to the address listed.
